I know a lot of HTML (though I learned it about ten years ago) and some JavaScript, but next to nothing about CSS and XHTML. What's a good book for me to read to get myself up-to-date?

I'm not sure what you mean by 'intermediate', but if you're already familiar with html and CSS best practices, then I would recommend Bulletproof Web Design by Dan Cederholm and Transcending CSS from Andy Clarke.

I'm looking to learn web design. Whilst researching the subject, I gathered that I should learn HTML, XHTML and CSS. Which book would you recommend to a complete beginner?
